- obtain relevant previous imaging, imaging reports, pathology results and other clinical information where reasonably necessary for your care;
- manage appointments, referrals, communications, billing and payments;
- contact you regarding your care, results, follow-up or enquiries;
- undertake clinical audit, quality assurance, accreditation and service improvement; and
- meet our legal, regulatory and professional obligations.
Where we obtain information
We usually collect information directly from you. Where reasonably necessary for your care, we may also obtain relevant information from your referring practitioner, general practitioner, other healthcare professionals, hospitals and health services, diagnostic imaging or pathology providers, BreastScreen services, Medicare, DVA, insurers or another person authorised to act on your behalf.
Who we may share information with
Where reasonably necessary for your care, we may provide relevant personal and health information, including medical images and reports, to your referring practitioner and other healthcare professionals or health services involved in your care.
We may also disclose information to organisations that support the operation of Canberra Breast, including providers of medical imaging and reporting systems, information technology and secure data hosting, billing and payment services and other professional or technical services. Information may also be disclosed where required or authorised by law.
Your privacy rights
You may request access to, or correction of, personal and health information Canberra Breast holds about you. You may also contact us if you have a privacy enquiry or complaint.
If you do not provide information that is reasonably necessary for your care, Canberra Breast may be unable to provide some or all of the healthcare services requested.
